Wellness & Reablement

This workshop is designed to build direct care staff’s understanding and confidence about what person centred practice looks like in practice and how to work with people’s goals and care plans to deliver care that is individualised and effective. Key topics will include

Wellness and reablement in aged care … what does it take?
Key skills, practices and approaches that support wellness in action
How individual goals support a wellness approach
How to read and use client care plans to guide your work with clients and carers
Working collaborative with clients and carers to promote their independence and assist them to achieve their goals.

A range of examples and case studies will also be used to demonstrate how these can be applied within direct care and build their skills and confidence to embed these skills in their day to day work
