Social connection and support from friends, family, and community can make a world of difference at the end of life. The Healthy End of Life Planning (HELP) App is a free, easy-to-use tool that helps people coordinate care and build strong support networks.
Join this free, hands-on workshop to explore how the HELP App works, the research behind it, and how you can play a vital role in network-centred care for people in your community.
The Workshop will include:
Insights into the research behind the HELP App
Understanding the barriers around supporting someone at end of life and how to address them
Exploring the concept of network centred care
Overview of the HELP App and the key features
